The permit application forms listed below are provided in the Microsoft Word document template format (*.dot). You will need Microsoft Word to open and fill out the forms.  These forms may be downloaded for the purpose of applying for an air pollution permit in Hamilton County and its municipalities.  These forms may be filled out in Microsoft Word or printed blank and filled out manually.  To download a form, right-click on the link and select "save target as".  You may then save the form to your hard drive.  To fill out the form using the Microsoft Word plug-in in your browser, left-click on the link and select "cancel" when prompted for a password.  The "save as" command under the "File" menu may then be used to save the form to your hard drive.
